Alaska Death Certificates

Initially you need to fill out the Alaska Death Certificate Request form, which you can get through the state's website, specifically that of Alaska's Bureau of Vital Statistics. You must be among the people who are eligible to purchase a copy of the death certificate. These people are the spouse, parent, sibling, or child of the deceased.

For identification purposes, you have to include a copy of a government-issued picture ID. For your convenience as well as the office's, make sure that the copy is clear and, if possible, enlarged so the identification process won't be too difficult. The copy must have a signature of the applicant.

Generally, each certified copy of the certificate will cost $20. However, there will be additional charges depending on the circumstance, such as when you pay with a credit card (additional $11) or when you need to have the certificate authenticated for a foreign country ($12).
