The apply for a California death certificate you need to fill out an application form and send it to the following address: Office of Vital Records - MS 5103 The application form is available for download at California 's Department of Public Health website. If you intend to get an authorized copy, you need to have a notarized statement that you are requesting for an authorized copy. Otherwise, you just need to fill out the application form. Most of the fees are varying, but the death certificate only costs $12 (varying prices for specific death certificates such as those for still birth, etc.). Don't expect to get your certificate the next day after you send your application. Also, make sure you don't send any cash with your in-mail application.
