You may either file your request for a certified copy of a death certificate by going online or make the request through our form at the top or bottom of this page, or you can mail your application by writing a letter and attaching the request form, identification and the check or money order for payment t this address: Idaho Bureau of Vital Records and Health Statistics With respect to fees, death certificates cost $13 each. If you ordered via fax or with a credit card, then there will be an additional charge of $10.50. If you ordered with Express Service specified, then there will be another additional charge of $19.00. It will take around 5-7 business days after receipt for a mailed application sent to the Vital Records and Health Statistics Office to be completed. But if you made the order via fax or online and paid for return services via Express Service, you will get your certificate within two days after the office receives the request.
