INDIANA Public Death Records

You can request for certified copies of a death certificate of a person who died within the state of Indiana either from the state Department of Health or at the local health department of the county where that record was filed. In either case, those who are requesting for these certificates must supply a proof of relationship with the deceased and proper identification of the one making the request.

Death records in the Vital Records office date back to 1900. For requests on certificates filed before that, you need to go to the local health department where the death occurred. In addition, requested certificates on deaths which took place between 1900 and 1917, you need to supply information on the city or county of death in order to be able to locate that record.

Indiana Death Certificates

Generally, you will need to supply a photo copy of one valid photo ID or two of the alternative documents (photo copy as well) to comply with the identification requirement. The relationship proof requirement will depend on who you apply as. For instance, if you apply as the legal guardian of the deceased, then you have to submit a document proving your guardianship status.

For applications, you need to print out form 49606 or the Application for Search and Certified Copy of Death Record. This form can be downloaded from the State's Department of Health Website.

If you intend to apply for the certificate in person, then you can go to the ISDH Vital Records Office, which is located at 6 West Washington Street, Indianapolis, Indiana 46204. This office will be accepting walk-in requests between 9:00 AM to 2:00 PM from Mondays through Fridays (except for official holidays). Each first certificate will cost $10, and any additional copy requested for at the same time will cost $4. If you apply for the certificate in person, you only need to wait about an hour to get it.

You can also apply by mail, making the payment via money order or check payable to Indiana State Department of Health. The certificate will cost $8 and additional copies $4. Processing time will be 3-4 weeks from the day the office receives the request.
