You will get most of the information with respect to getting certified Iowa death records filed in its respective Health Statistics Bureau. Anyone who wishes to apply for a copy of these records by phone may call up the office at (515) 281-4944. Calls are accepted between 7:00 AM and 4:45 PM. between Mondays and Fridays, with the exception of official state holidays. When you make that call, a recording will provide you with instructions and guide you throughout the application process. They also provide pre-recorded instructions with respect to making applications via mail, etc. If you choose to pay using a credit card, you will also be informed of the applicable charges ($9 for the screening and authentication charge plus $15 record fee). For applications for certified death records filed with the Iowa Department of Public Health through mail, you need to fill out the Application for Copy of a Vital Record form. Include a check or money order payable to the Iowa Department of Public Health and a photocopy of your government-issued photo ID with the completed form and send it to this address: Iowa Department of Public Health Each certificate applied for will cost $15 – this covers the search fee as well as one copy. Additional copies will also cost $15. Applicants can also request for that copy in person at the office between 7:00 AM and 4:45 PM from Mondays through Fridays, except on holidays. The office is in the Lucas Building, located at the same mailing address mentioned above. It's the first one east of the Capitol Building and south of Grand Avenue. Walk-in applicants must also provide the same required government-issued photo ID. After applying for the certificate, you can pick up the record after two days or have it mailed to you.
