For applications for certified death records filed with the Iowa Department of Public Health through mail, you need to fill out the Application for Copy of a Vital Record form. Include a check or money order payable to the Iowa Department of Public Health and a photocopy of your government-issued photo ID with the completed form and send it to this address:

Each certificate applied for will cost $15 – this covers the search fee as well as one copy. Additional copies will also cost $15.

Applicants can also request for that copy in person at the office between 7:00 AM and 4:45 PM from Mondays through Fridays, except on holidays. The office is in the Lucas Building, located at the same mailing address mentioned above. It's the first one east of the Capitol Building and south of Grand Avenue. Walk-in applicants must also provide the same required government-issued photo ID. After applying for the certificate, you can pick up the record after two days or have it mailed to you.
