The State of Kansas authorizes by law the Department of Health and Environment to issue certified copies of death records, among other vital records. The office strictly does not provide uncertified copies nor issue verifications to the public. Aside from that, the initial $13 certificate fee is also provided by law to serve as a five-year search fee. Walk-in applications for death certificates are a priority service. Applicants who choose to apply for the certificates this way can go to Curtis State Office Building , 1000 SW Jackson, Ste.120, Topeka , KS 66612 -2221. The Customer Service Lobby accepts these applications between 9:00 AM and 4:00 PM, on weekdays, excluding holidays. An application form must be filled up. You can either choose to download it off the State's Department of Health website, fill it up and bring it with you or simply fill out the available forms at the office. You can also opt to apply for the certificate via telephone. You can dial (785) 296-3253 on weekdays between 8:00 a.m. and 4:00 p.m. Payment made using this method must be in credit card, and the card you are using to pay the certificate for should be in your name. In addition, you will also be requested to provide either your social security number or your driver's license number. Accepted credit cards include Visa, MasterCard, Discover, or American Express. Applications in this manner will take around 3 business days to process. You can also apply using regular or priority mail. Just send your application as well as the other requirements via mail to the State's Department of Health office and you will get your certificate within 5-10 business days for regular mail and 3 business days for priority mail.
