KENTUCKY Public Death Records

Kentucky's Cabinet for Health and Family Services keeps a file of all death certificates since 1911 up to the present. Kentucky death records can be ordered and paid for either by phone, online, by mail, or in person by going to the office. If you will be making your request via mail, then you will have to wait around 30 working days to get your request processed, just like all the other record types that are filed in the office.

Under state law, the Kentucky Office of Vital Statistics cannot give out information from records for free, do a genealogical search or see if a certain record is on file unless an application form is completed by the one making the request and he or she pays the applicable charges. After the required application form is completed and the fees are duly paid, the Vital Statistics Office will search for the record and mail it to you if the file is found. The fees that were paid will also cover this search and in cases where the certificate cannot be found you do not get your payment refunded. Also, the office is not allowed to give out copies which are non-certified or photo-copies of these certificates.

KENTUCKY Death Certificates

To apply for these certificates you need to supply some information. First you have to give the full name of the deceased, the month, day and year of death, and the county where the deceased had died. You also need to supply the following information so that you can easily get your certificate: your name and the address where the certificate should be sent to via mail, the phone number or contact number where you could be called on any time between 8:00 AM and 4:30 PM, and the number of copies you are ordering.

After supplying this information, attach this with the application form along with a check or money order and have it payable to the State Treasurer of Kentucky. Mail all this to the address below:
