This office collects records of deaths within the state from the year 1892 to the present. You will be guided through an automated telephone system. You have to pay via credit card. In either case, when asking for a death certificate from the Maine Vital Records, you need to supply the following information: Name of the person whose death certificate you are requesting, the date of his or her death, the city or town where the death occurred, your relationship with the deceased, your contact number which you could be reached during the day, and a check or money order payable to “Treasurer – State of Maine”.
