Massachusetts maintains in its Vital Records and Statistics Registry a file of death records which were dated at any time from the year 1916 to the present. These records have been registered in the state since 1635. The statewide collection of these records began in 1841. Thus, events that were recorded from 1841 to 1915 can be found at the state archives while records dated earlier than that can be found at the proper office of the city where the event occurred. You can apply for a copy of a Massachusetts death certificate in person by visiting the Registry at any time between 8:45 AM – 4:45 PM on weekdays, except holidays. The registry is located at this address: 150 Mount Vernon St., 1 st Floor The records requested for in this case will cost $18 each, which covers a ten year search of that record if the date and place of the death is unknown. You can also go to the research area of the office, where all the records are public. You can also use VitalChek and order the first copy online at the cost of $45 and $37 for every additional copy ordered at the same time. The order must be placed with a major credit card. Orders via the internet will be processed in 7-10 business days, but for an additional $15 if you can have the record shipped using Federal Express Overnight. For telephone call orders, dial (617) 740-2606. Costs for the copies are similar to the charges if ordered online, but the Federal Express Overnight fee is $15.50. You have to pay with your credit card. The same goes for fax. If you want to apply via mail, fill out a mail order form (downloadable at the state's registry website) and mail it to the address mentioned above. Include a self-address and stamped business size envelope and your check or money order, and make it payable to “The Commonwealth of Massachusetts”. Records ordered this way cost $28 and are processed within 20-30 business days or $37 if you use the expedited mail service, which will take around 10 business days.
