MISSISSIPPI Public Death Records

Mississipi's State Department of Health maintains an archive of death certificates with records dating from 1912 through its Vital Records Office. This is different from the Vital Statistics office where you can find statistical reports on deaths within the state. Records dated prior to 1912 can be found at the Department of Archives and History.

This Vital Records Office is located at Underwood Building , 571 Stadium Drive, Mississippi . It's near Woodrow Wilson Avenue , right at the North State Street . It's open on Mondays to Fridays from 8:00 AM to 5:00 PM.

MIssissippi Death Certificates

To order certificates by mail, you start off by downloading and completing an Application for Certified Death Certificate, found at the state's health department website. The application must be printed, completed and signed. You must also include in your application a copy of your valid photo identification, such as your driver's license or any state-issued photo ID. There are additional identification requirements depending on the person applying for the certificate. Mail the application to the address below:

The initial ordered copy costs $15, and subsequent copies requested at the same time cost $5. Your payment should be sent along with the application. Payment should be in personal check, money order, or bank cashier's check payable to Mississippi Vital Records. This payment covers the five-year search for every application.
