The search fee for death records are $13 per record and $10 for additional copies requested at the same time. When you intend to pay by check or money order, make sure that it is made payable t the Missouri Department of Health and Senior Services. If you intend to use a check, make sure that it is drawn by a United States bank. Money orders may either be issued by a United States bank or the United States Postal Service. You are not allowed to send cash. After filling up the application you can either take your application to the local health department near you or mail that application to this address: Missouri Department of Health and Senior Services Bureau of Vital Records Along with your application and payment, place inside your mail a self-addressed and stamped envelope (legal size). Once your application is received your payment will be deposited immediately. Take not that applications without fees will not be processed and be returned to you. Your application will be processed within two to four weeks. You can also request for a vital record without an application form. You can do so by writing to the Bureau of Vital Records to the address mentioned above and including the following information: full name of the deceased, date and place of death, name of the parents of the deceased, and your relationship with the deceased. In this case you will also be required to send payment via check or money order, made payable to Bureau of Vital Records.
