The mail application forms can be downloaded at the website of the Montana Department of Public Health and Human Services. Mail applications are normally processed in a matter of two weeks. For follow-ups on the status of your request you can dial 406-444-2685.

As for applications by fax, you need to pay using your credit card. Download the fax application forms from the website, fill them up and fax them to 866-696-1912 (toll free).
