NEBRASKA Public Death Records

Death certificates filed in the Nebraska Department of Health and Human Services are those records which involve deaths since 1904. According to Nebraska statutes legal copies of death certificates are only issued to applicants who bear a proper purpose. These purposes may vary depending upon who are applying for the certificate, but they usually involve legal matters or for purposes of research.

But if you can't provide proof of such purpose, then you are not eligible to get a legal copy of a death certificate. So make sure that you provide a proper purpose when you request a death certificate. If you are the spouse, parent, sibling or legal representative of the deceased, you can also ask for a legal copy of his or her death certificate. There will be a fee which will cover a copy of the certificate and a search for that record. If no record is found or that you failed to provide a proper purpose, you will be notified accordingly.

NEBRASKA Death Certificates

If you want to make your request by mail, include with your application an $11 money order or check made payable to Vital Records. You can't pay by credit card.

Aside from the proper purpose, you must also provide important information about you as well as the person whose name is on the death certificate. This includes but is not limited to your Name and relationship with the deceased as well as the deceased's personal information.

After completing the above requirements, mail it to this address:

If you want you can also make use of the Electronic Death Registration System that the Vital Records Office uses. This is a system that electronically registers death records to the proper Count Registrar so that it will be easier for deaths to be registered and recorded and that these records will be much easier to access.

The office provides tutorials on how to use this electronic system for County Attorneys and Physicians, covering phases from recording death records to accessing their corresponding certificates.
