NORTH CAROLINA Public Death Records

Requesting for North Carolina Death Records requires the person making the request to take and complete a standard form, which must be signed and sent to the proper office in order to properly document all incoming requests for copies of these death certificates. These forms can be downloaded from the website of the Vital Records Center of North Carolina.

You need to supply the following information in the application for a death record:

•  Full name of the person whose name is on the death certificate being requested;
•  The date the deceased died;
•  The City or County where the deceased died;
•  The deceased's date of birth; and
•  Your relationship with the deceased.

If you are not an immediate family member of the deceased and need a copy of the death certificate, you must provide briefly the reason why you need to acquire the document in lieu of the former requirement.

The above information is very important if you are requesting for a certified copy of the death certificate. North Carolina law specifies strictly who are entitled to get a certified copy. These copies are deemed as legal documents and can be used for various legal transactions. On the other hand, uncertified copies are open to the rest of the public but cannot be used for the same exclusive purposes above and are at best meant for informational purposes.

NORTH CAROLINA Death Certificates

For every document searched in a three year period, applicants are charged a fee of $24, while additional copies requested for at the same time are $15 each.

Mail requests must include, along with the application forms and other requirements, a money order or check for the fees to this address:

The office is open on weekdays from 8:00 AM to 4:00 PM, except holidays. The applicable fees are the same, except that if you want to get your document within the same day you have to pay for the $15 expedited service.
