NORTH DAKOTA Public Death Records

Since January 1, 2008, the Vital Records Division of the North Dakota Department of Health has made a lot of changes on their procedures for requesting for death records, and all of which are based on legislative changes that were made on the year 2007.

For one thing, the Vital Records Division will now be requiring proof of identity from all persons making requests for a death certificate from the office. Identification is established by presenting either a copy of one among a state-issued photo ID, driver's license, tribal ID, military ID, or passport or Visa. If you don't have any of the above, then you can also present a copy of two of any of the following: Social Security Card, utility bill, bank statement, pay stub, car registration, or IRS Tax Return.

NORTH DAKOTA Death Certificates

Also, the Office now issues three types of documents:

  • Certification of the Facts of the Death – issued after the funeral home registers the facts of death with the office. It does not have much demographic regarding the deceased, but contains the social security number, which is important for various transactions such as closing the deceased's bank accounts.
  • Certification of Death – issued after the medical aspects of the record are completed. It's like the first type only that it contains the cause of death of the deceased. This type of document is also necessary for many legal transactions such as insurance claims.
  • Informational Copy – for informational purposes only. It is not considered a legal document and does not contain the cause of death and social security number.

Except for the third type which can be issued to the general public, these certificates may only be issued to a person authorized by law to make a request such as the immediate family member of the deceased or his legal representative.

Each first copy will cost $5 and additional copies ordered at the same time will cost $2 each. Copies can be requested by mailing to the following address:

You can also make the request in person by visiting the office at the location above.

For fax requests through (701) 328-1850 and internet requests through the Department of Health Website, payment is made via credit card.
