OHIO Public Death Records

Ohio assigns the filing and recording of death records along with other vital records pertaining to events which took place within the state to its Department of Health. They can be contacted for questions and inquiries by dialing (614) 466-2531.

There are five different ways for you to obtain a copy of a death certificate from Ohio , which will be discussed below.

You can get a copy of a death record from the local health department of a city or county where the death occurred. This could be more convenient than going to the main state office. Fees may vary for every county or city.

OHIO Death Certificates

But if you want to avail of the same day service (meaning you get the certificate on the same day you made the request), you can go to the office located in the address below:

Except on holidays, the office will be open from 8:00 AM to 5:00 PM on Mondays to Fridays. The waiting time is around 30-60 minutes. However, walk-in requests that are made beyond 4:30 can't be guaranteed to be processed on the same day. You can also choose to have the record mailed to you (within one week) or that you come back for it at a later date.

For mail-in requests send a letter requesting the certificate to the following address:

The request must include the following information: Name of deceased, date and place of death, contact number where you can be reached during the day and the address where the record will be mailed to. Payment must be sent along with the application through check or money order made payable to Treasurer, State of Ohio . You will receive your certificate in 2-3 week.
