The Oklahoma State Department of Health Vital Records Service is in charge of keeping file of all certified copies of Oklahoma death certificates. These documents can be requested either by writing to or visiting the office or any of its locations. Death certificates filed in Oklahoma are not open to the public. It is only through a proper application containing proper information and accompanied by identification and applicable fees can an individual obtain such document. Providing the necessary information will allow the office to better help you in processing your request. The information includes the full name of the deceased, his or her date and place of death, your relationship with the deceased, name of the funeral home that delivered the service, your handwritten signature and your return address. You also need to present a valid photo ID. By giving a complete set of information, you make your own application convenient because searching will take less time and there will be little or no chance for delays. To request for a death certificate you need to fill out an application form and provide the information above. Send it, along with the fees to the address below: Vital Records Service Or you could personally submit the application to any of the following addresses: Vital Records Service, Room 117 Or Tulsa Health Department, Or Pittsburg County Health Department Make sure that you deliver the application between 8:30 AM and 4:00 PM between Monday and Friday. Each certified copy of a death certificate costs $15. For mail-in applications, include in your application a check or money order, made payable to Vital Records Service. Also include a self-addressed and stamped envelope for the return of the certificates. Mail-in applications will take at least 4 weeks to process while walk-in applications can take at least an hour. If you use US Postal Service, however, processing time for mail-in applications may be reduced to 24-48 hours for additional charges.
