It is the task of the Center for Health Statistics of the State of Oregon to issue certified copies of vital records, which includes death certificates for deaths that took place within the state. Oregon death records that are on file are dated back to the month of July of 1903. In order to make a request for a death certificate, you must provide the following record information: full name of the deceased and his or her spouse, the date and place (county) where the deceased passed away, the applicant's relationship with the deceased, and the purpose for acquiring the record. For the first 100 years from the filing of the death certificate, such record is kept confidential and is only available to the immediate family members of the deceased and his or her legal representatives. There are also other persons and entities specifically provided by law to be authorized to make such requests, such as government agencies that need the record for their functions. For mail-in applications send your record information along with your return address and daytime contact number to Oregon Vital Records, PO Box 14050 , Portland , OR 97293-0050 . Each initial copy costs $20 and additional copies ordered at the same time cost $15. Payment must be made either via check or money order payable to DHS Vital Records. There are rush services available at additional charges. Turnaround time is 5 weeks. You can also order the certificate online by filling the form at the top or bottom of this page. Our system will perform a preliminary search of our database. Fax requests may be sent to (503) 234-8417. Requests must contain the record information along with a return address, daytime contact number, and your credit card number with its expiry date. Orders by telephone can be made by dialing (971) 673-1190. The office may be called between 8:00 AM to 4:30 PM from Mondays to Fridays. For fax and telephone requests the charge is $32.50 and the order is processed either the same day or the next working day. Lastly, you can visit the Vital Records Office in person in Portland , at 800 NE Oregon Street , Room 205. You should come on weekdays from 8:00 AM to 4:50 PM. You only need to wait for around 15-20 minutes to get the record. Don't forget to bring with you the record information.
