RHODE ISLAND Public Death Records

The Department of Health Vital Records Office of Rhode Island is the one tasked with the fling and issuing of certified copies of death certificates. This is where you can generally apply for a copy of any death certificate that is kept on file.

But aside from the office, you can also approach the town or city hall where the death recorded in the certificate you are requesting for occurred to get a copy. Also, death records which have lapsed 50 years since it was filed become public records and therefore can be obtained at these city or town halls or through the Rhode Island State Archives.

As for later records, they remain confidential. As state law would provide, the only persons who can apply for a copy of the death certificate are those who have a tangible interest in it. These persons are the immediate family members or guardians of the deceased, attorneys representing the deceased and his estate, and third persons who have legal interest in acquiring the certificate.

rhode island Death Certificates

Those requesting for a copy of the death certificate may either apply for it by mail or in person.

For mail-in applications, you have to download the application form found at the Vital Records Office's website and complete it. Attach with it a copy of a picture identification of yourself (i.e. passport, driver's license, etc.) or one copy of each of two documents that show your name and address (i.e. utility bill, bank statements, income tax return). And then include a check or money order payable to General Treasurer, State of RI, for the payment. Do not send cash.

Generally you will be able to get your certificate in 4-6 weeks time. You can ask for priority service for an extra $7.

You can also visit the same address above and make the application in person. Bring all the requirements and present them there. The initial certified copy will cost $20, which includes the searching, while additional copies ordered at the same time are $15 each.
