You can also make your request by mail, by sending an application to this address: Office of Vital Records and Statistics Along with the completed form you must provide proof of identification as required by Utah state law as well as a check or money order payable to the Vital Records. Mail requests are completed within 3 weeks. You can also apply for the certificate in person by visiting the office located in Salt Lake, specifically at this address: 288 North 1460 West, Salt Lake City, Utah. You may visit the office from Mondays through Thursdays, between 7:00 AM and 6:00 PM. If you made your application after 5:30 PM, then it will be processed by the next business day. Keep in mind that the office only issue certified copies of the document. Certified documents have a raised seal at the bottom right hand corner of the document and the document is printed in multi-colored paper. These documents are deemed as legal documents and can be validly used to complete various legal transactions.
