Death certificates in Washington can be obtained from three different offices who file them, and which office you need to go to will depend on what year the death record you are looking for was filed. For records of deaths that occurred within the past three months, you must visit the local health department where the death occurred. If you're looking for a death certificate that's beyond three months old but was filed after July 1, 1907, visit the Center of Health Statistics of the Department of Health of Washington. There are also different ways for you to obtain a copy of a death certificate from the Vital Records Office. The easiest way is by availing of the walk-in same day service. You can visit the Vital Records Office between 8:00 AM and 4:00 PM from Mondays through Fridays, except during state holidays. The office is located at this address: Center for Health Statistics Download an ordering form from the website of the Department of Health office, complete it, and bring with you to the office or you can just fill out one that's available in the office. You will get your certificate within the day you've applied for it. You can also place your order by mail, which only takes 3 to 4 weeks for it to be processed and delivered. Download the same application form above and complete it. Send the form, along with a check or money order to this address: Center for Health Statistics For both walk-in and mail-in services, make your check or money order payable to the Department of Health. You can also order by phone through (360) 236-4313 or (360) 236-4312 or by fax through (360) 352-2586. Your payment must be done through credit card. For walk-in and mail-in services, certificates cost $20 each and copies ordered by fax or phone are $32 plus additional fees for expedited delivery.
