According to Wisconsin law, it is required that all requests for copies of death certificates have to be made with signed application forms and paid with the appropriate fees. Therefore, the Vital Records office does not process applications online or by telephone, regardless of the kind of copy which is being requested. You can, however, apply for a copy of a death certificate by person, mail or fax. To apply for a certificate in person, visit 1 West Wilson Street , Room 158, in Madison , Wisconsin from 8:00 AM to 4:15 PM on any weekday except holidays, and make your request there. You can either choose to bring a filled out application form with you (download it from the website and print it out) or you can fill one up at the office. You will be required to present a valid identification, preferably a valid photo ID that shows your name and current address. The identification document must not have expired. Walk-in applications will take around two hours to process at regular service. Expedited service takes 15-30 minutes.
